Foster Family Agreement

I want to help Circle Tail, Inc. raise and train well-socialized, well-mannered assistance or facility dogs (or, in certain circumstances, provide care for a breeding dog). As a Circle Tail Foster Family/Parent, I agree to adhere to all the regulations and requirements of Circle Tail or I will relinquish the dog back to Circle Tail and discontinue fostering. If the dog is not thriving in my home, Circle Tail may request the dog be returned. I will foster the dog for periods of time as determined by Circle Tail and me. While the dog is in my care, I will follow the guidelines in the Foster Family Manual, socialize the dog to as many sights, sounds, people, and places as possible, reinforce the commands the dog already knows, reinforce good house manners, and report to Circle Tail the outcome of the dog's outings. The Circle Tail staff will be able to assist me with any concerns or problems I may have while fostering the dog.

I acknowledge and agree to the following regulations, responsibilities and commitments of a Circle Tail Foster Family/Parent:

The dog I am caring for belongs to Circle Tail, Inc.
I will be financially responsible for treats and toys for the dog while he/she is in my home.
I will give the dog exercise, training and play time, attention and alone time.
I will foster the dog in my house and depending on age and behavior, the dog will sleep in a crate or on the floor in the bedroom or as discussed/approved by Circle Tail. I will not allow the dog to sleep on the bed.
The dog will have house training and in-house etiquette consistently reinforced. I will not allow the dog on any furniture, to beg for food, to jump on people at any time, or to mouth people. I will not allow any "rough housing" with the dog, playing tug-of-war, or chasing of the dog.
I will only allow the dog to interact/play with other pets under supervision during limited times. I understand the dog must be encouraged to interact with humans.
I will only allow the dog to interact/play with children under close supervision.
I will familiarize the dog with people of all ages and varied places and sounds. I will work on confidence in new situations and calm greetings with new people.
The dog will be inside my house in a crate or other safe area inside my house (as determined by dog’s behavior and age) when I am not home.
I will take the dog with me on short car trips and reinforce proper entry and exit from the vehicle.
I will only take the dog into public building/facilities after approval by Circle Tail. If approved, I will always have the dog on leash and under control and, in non-pet friendly establishments, in his/her vest.
I will carry my Community Trainer ID card with me when on outings and educate the public as necessary on proper assistance dog etiquette in a polite and professional manner.
I will not allow the dog to be petted while in vest and I will encourage proper “in public manners” such as no sniffing or eating.
I understand the law regarding public access for trainers with assistance dogs in training and will not take a pet or therapy dog out into public where pets are not permitted.
I will use only toys approved by Circle Tail.
I will feed the dog according to Circle Tail specifications, and I will not feed the dog any table food nor allow him/her to beg for food, eat food off the floor or become overweight.
I will reinforce the dog being handled for grooming, teeth brushing and nail trimming.
I will walk the dog regularly on a loose leash and not allow the dog to pull on the leash.
I will not allow the dog to be off leash outside unless in a safe, fenced in yard. I also will not tie the dog outside unattended.
I will notify Circle Tail immediately if any injury or illness should occur while the dog is in my care. I will obtain emergency veterinary care for the dog if necessary.
If I am no longer able to foster dogs in training for Circle Tail, I will return all borrowed items and will relinquish my Community Trainer ID card back to Circle Tail.
I agree to assume full legal and financial responsibility for any and all property damage or personal injury caused by the dog during the time that s/he is in my care and to indemnify and hold Circle Tail harmless from any liability or claims arising out of or relating to this agreement or any damages or any personal injury caused by the dog, including, without limitation, claims for myself, my heirs, successors, spouses, agents, executors, legal representatives, administrators, assigns, and/or any claims by any third parties.

Acknowledgement

The information on my application is true and accurate. I have read the above information and agree to these responsibilities and obligations and fully understand them. I also understand that if this agreement is not adhered to, among other things, the puppy/dog may be removed from the premises and placed elsewhere.
